What is the purpose of the Form 4868?

The purpose of the Form 4868 is to request an automatic 6-month extension to file the U.S. individual income tax return. Note that an extension of time to file is for the income tax return only, not an extension of time to pay any taxes that are due. In order to avoid penalties and interest, any taxes due must be paid on, or before the April 15th due date. If the 15th falls on a recognized holiday or a weekend, the due date is the following business day.

How is the Form 4868 completed?

Form 4868 is a very short form. In consists of two parts. Part I (Identification), and Part II (Individual Income Tax). The form is completed as follows;
Part I: Identification
Line 1: Enter your name and address
Line 2: Enter your social security number
Line 3: If you file jointly, enter your spouse’s social security number here
If you plan to file a joint return, information for both spouses must be entered. The names must be entered in the order that they will appear on the tax return.
Part II: Individual Income Tax
Line 4: Enter the estimate of tax liability for the current year.
Line 5: Enter the total payments you expect to report on your current year’s tax return.
Line 6: Enter the balance due. Subtract line 5 from line 4.
Line 7: Enter the amount that you are paying with the extension.
Line 8: Check the box on this line if you are outside of the U.S. on the regular due date of the tax return.
Line 9: This line is for 1040NR or 1040NR-EZ filers. If you did not receive wages subject to U.S. income tax withholding, and your return is due June 15th, check the box on this line.
